Telehealth - healthcare at home

One of the main benefits of telehealth is that it can move the focus of patient care from the hospital into the home. It is often used for patients with chronic diseases and can be beneficial for other at-risk groups, such as the frail elderly or some people with disabilities. The mobility limitations experienced these groups can make transportation to health appointments an ongoing issue. By enabling care professionals to monitor a patient's health and well-being from a distance, telehealth can also encourage improved self-management of their condition. Other benefits include an improved quality of life, and less need for expensive inpatient hospital stays and visits to the emergency department.

Telehealth systems

Telehealth systems generally consist of a home unit where vital sign readings are taken by patients. Readings are then transmitted to a central hub in a clinic or monitoring centre such as that operated by TASK Community Care. The physiological data is presented on screen to a telehealth nurse or other care professional through a central hub. If readings go outside pre-set parameters they will be automatically flagged on screen, allowing timely and appropriate care decisions to be made. This could involve the telehealth nurse giving the patient a phonecall to check whether or not further action needs to be taken.

TASK telehealth

In addition to arranging the supply of your telehealth equipment, we can provide training, installation and on-going technical support. A short trial using the equipment will be possible in certain cases, giving you the opportunity to test for suitability and functionality prior to implementing a pilot or complete telehealth/telemedicine program.
